Care Coordination Part 1

In this first episode of our three-part series, we’re breaking down one of the most important yet overlooked pieces of Value-Based Care: Care Coordination. With the tendency to operate in silos, many clinicians struggle to tackle a holistic and individualized patient experience. We’re joined today by experts across the care spectrum to better ascertain how to utilize technology like patient monitoring, community partnerships, and teamwork with clinical and non-clinical staff to put patient experience at the forefront of VBC. We also discuss the roadblocks that make effective care coordination difficult, including social risk factors and how to build systems to support the infrastructure of quality care.
